Our Hospice Team

A hospice team is made up of a group of compassionate, specially trained professionals who work together to provide holistic care and support for patients and their families.

Here’s who typically makes up the team:
 
  • Hospice Physician or Medical Director – Oversees the patient’s care plan and works closely with other medical providers to manage symptoms and ensure comfort.
  • Registered Nurses (RNs) – Provide hands-on care, assessments, monitor the patient’s condition, manage pain and symptoms, and offer education and support to family members.
  • Licensed Practical Nurse (LPN)– Carries out patient monitoring, comfort care, medication administration,  emotional support, and documentation.
  • Certified Nursing Assistants (CNAs) – Help with personal care needs such as bathing, grooming, and dressing, promoting dignity and comfort.
  • Social Workers – Offer emotional support, help with practical needs like advance directives, and connect families to community resources.
  • Chaplains or Spiritual Counselors – Provide spiritual support based on the patient’s and family’s beliefs and values, regardless of religious affiliation.
  • Bereavement Counselors – Support families during the grieving process, both before and after a loved one’s passing.
  • Volunteers – Trained individuals who offer companionship, respite for caregivers, and support with errands or other non-medical needs.
 
This team works together to ensure that care is not only medically effective, but also emotionally and spiritually meaningful.
